Upon Arrival

Please arrive 10 minutes prior for your first visit. This will allow time for our therapists to prepare you for your treatment and for you to complete any necessary paperwork.

Eden Street Salon and Day Spa Client Medical History Form

If you are new to Eden Street Salon and Day Spa, you will be asked to fill out a confidential client’s Medical History Form.

This form provides our service providers with the information needed to administer the treatments safely and without complication. Conditions such as pregnancy (or the possibility thereof), recent surgery, or various other conditions can conflict with certain treatments and it is in your best interest to complete the necessary medical history form.

All information collected is strictly confidential and is only shared with the therapists who will be performing your treatment.
